Care Partner Lead - Support at Home If you’re an experienced Care Partner ready to lead, influence practice, and shape great client outcomes under Support at Home - this is your next step. The opportunity: As our Care Partner Lead, you’ll be a senior member of the Home Care team, leading great care management: smooth onboarding, strong care planning, responsive reviews, and clear budget oversight. You’ll support and guide care management teams across Adelaide and the Fleurieu Peninsula, helping everyone do their best work - consistently and compliantly. This role is ideal for a high-performing Care Partner who’s ready to lead from the front, coach others, and step into broader responsibility. What you’ll be doing: - Coach, mentor and support a geographically dispersed team to build capability and consistency in care management
- Lead and oversee client onboarding so services start on time and safely
- Guide and strengthen care planning, reviews, and coordination for complex and high-risk clients
- Support consistent, high-quality documentation and follow-through in systems
- Monitor care management activity and help the team manage time, priorities, and workload
- Oversee client budget management and support transparency and value-for-money decisions
- Set strong customer service expectations across reception, phone/email enquiries, and follow-up
- Contribute to compliance, governance, quality improvement, and audit readiness
What you’ll bring - Solid home care / community care experience (aged care or NDIS)
- Demonstrated leadership experience (ideally 3-5+ years supporting teams)
- Strong judgement - you know when to act, when to escalate, and how to navigate complexity
- Clear written communication and strong documentation habits
- Confidence working with funding, service agreements, and client budgets
- A practical, warm, professional style with clients, families, and stakeholders
- A values-led approach: dignity, choice, inclusion, respect, and accountability
Essentials (clearances and requirements) - Certificate IV in Community Services (or higher)
- National Police Clearance
- Overseas criminal convictions statutory declaration (if applicable)
- Full, clean driver’s licence
- Ongoing commitment to annual flu vaccination
