We are seeking a compassionate, organised, and proactive Care Partner to join our Adelaide-based team on a full-time basis. About the Role

 

As a Care Partner, you will play an important role in supporting older people to remain independent, safe, and well in their own homes across metropolitan Adelaide.

This is a rewarding and varied role where no two days are the same. You will work closely with clients, families, and internal teams to help coordinate care, maintain accurate records, and ensure each client receives high-quality, person-centred support.

  • Support clients to access personalised, high-quality services in their homes
  • Build positive relationships with clients, families, and colleagues
  • Communicate clearly and professionally across a range of stakeholders
  • Maintain accurate client notes, records, and documentation
  • Use Microsoft Office and internal systems to manage information effectively
  • Work collaboratively within a close-knit team in a dynamic environment
  • You’ll be part of a fast-paced and collaborative environment, while still having the autonomy to manage your workload, prioritise effectively, and build strong working relationships.

Essential Requirements

  • Previous experience as a Care Partner is essential
  • Current driver’s licence
  • National Police Clearance

You are someone who is warm, approachable, and genuinely passionate about supporting older people to live well at home.

 

Strong communication and interpersonal skills

  • Confidence using Microsoft Office, including Word, Outlook, and Excel
  • The ability to stay organised and manage competing priorities
  • A positive, adaptable approach in a fast-paced environment
  • The ability to work well independently while also contributing to a team
  • A genuine commitment to person-centred care and quality service delivery
